Binswanger disease is a clinicopathological entity characterized by an ischemic periventricular leukoencephalopathy and mainly related to hypertension. Main clinical manifestations are cognitive impairment, parkinsonian gait disturbances, urinary incontinence, and pseudobulbar palsy. Brain MRI has improved our ability to make the diagnosis in vivo. Pathophysiology and therapeutic management are still under evaluation.
