Introducción
La hipertensión arterial pulmonar (HAP) se define como una enfermedad rara y progresiva, caracterizada por el incremento de las resistencias vasculares pulmonares, que en los estadios más avanzados da lugar a insuficiencia cardiaca derecha. Se diagnostica por una elevación sostenida de la presión arterial pulmonar media (PAPm) ≥ 25 mmHg en reposo1. Los síntomas de la HAP incluyen disnea, síncope y dolor torácico, produciendo, finalmente, la muerte prematura. Los cambios estructurales y funcionales
Pacientes y controles
La población de estudio consistió en 41 pacientes consecutivos diagnosticados de HAP idiopática y asociada del grupo i y 50 individuos sanos sin familiares conocidos con HAP procedentes de la población general. El diagnóstico de HAP se basó en un cateterismo derecho con una PAPm ≥ 25 mmHg y una presión de enclavamiento inferior a 15 mmHg sin tratamiento específico. Descripción de los pacientes
Cuarenta y un pacientes con HAP (17 idiopática, 16 asociada a conectivopatías, 4 portopulmonar y 4 asociadas al virus de la inmunodeficiencia) y 50 individuos control fueron incluidos en este estudio. En el momento del diagnóstico, 10 pacientes fueron clasificados en CF II, 28 pacientes en CF III, y 3 en CF IV. Al cabo de un año de tratamiento, 4 pacientes estaban en CF I, 16 pacientes en CF II, 16 en CF III, 3 en CF IV y 2 habían fallecido. En total, 18 pacientes fueron considerados buenos
Discusión
Este estudio ha mostrado una diferente distribución genotípica en el polimorfismo K198N del gen EDN1 en pacientes con HAP del grupo i en comparación con individuos sanos. La ET-1 desempeña un papel fundamental en la vasoconstricción y en la proliferación celular en pacientes con HAP.
