Introducción
La principal clasificación de la insuficiencia cardiaca (IC) se basa en la fracción de eyección del ventrículo izquierdo (FE)1. Aunque no es un parámetro ideal debido a su relativa subjetividad, la falta de evidencia que respalde el uso de otros parámetros ha hecho que la FE sea ampliamente aceptada para estratificar a los pacientes con IC. Diseño del estudio y participantes
Se realizó un estudio de cohorte prospectivo de pacientes ingresados en los servicios de Geriatría de 6 hospitales españoles con diagnóstico principal de IC desde el 1 de diciembre de 2010 hasta el 30 de noviembre de 20126, 7. Se incluyeron todos los pacientes con 75 o más años y diagnosticados de IC según los criterios de la Sociedad Europea de Cardiología y de Framingham8; además, para su inclusión, debían cumplir al menos uno de los siguientes criterios: índice de Charlson9 ≥ 3, dependencia
Características basales
Un total de 531 pacientes fueron incluidos y seguidos durante un año tras el alta. Las características basales se recogen en la tabla 1.
La edad media fue 85,5 ± 5,1 años, siendo el 63,7% mujeres. Es de destacar la elevada dependencia funcional (63,7% con índice de Katz B o más), comorbilidad (índice de Charlson 3,4 ± 1,9), deterioro cognitivo (46,4% con MEC < 24) y depresión (37% con escala de Yesavage > 5). Un 13,7% vivía solo.

Discusión
En pacientes muy ancianos ingresados por IC en servicios de Geriatría, un 10% tenían ICFEi. Estos pacientes eran similares a aquellos con ICFEr en cuanto a la menor edad y el predominio del sexo masculino; así como en la existencia de ingreso previo por IC, pero, en general, compartían características clínicas con ICFEr e ICFEc. Por otro lado, eran tratados de forma más parecida a los pacientes con ICFEr en cuanto al uso de fármacos para el bloqueo neurohormonal como IECA/ARA y betabloqueantes.
Conclusiones
En pacientes ancianos con IC, los categorizados como ICFEi no han mostrado claras diferencias respecto a aquellos con ICFEr o ICFEc, compartiendo características clínicas y sin observar diferencias en cuanto a la morbimortalidad.
