Aseptic Loosening – A US Perspective

Abstract

Aseptic loosening continues to be one of the leading causes of failure after primary total knee arthroplasty (TKA). Patients with aseptic loosening often express time-dependent symptoms. Aseptic loosening may present with pain and progressive instability that is worse with activity and weight-bearing, while improved with rest. Radiolucencies on radiographic examination typically begin on the femoral/tibial component periphery at the implant–cement interface and subsequently progress. It is important to correlate radiographic findings with the patient’s history and symptoms as not all radiolucent lines necessarily imply component loosening, especially if non-progressive. Revision TKA is a costly and complex surgical intervention that requires more resources than primary arthroplasty. Overall results of revision TKA for aseptic causes have historically been less predictable and with lower success rates and patient satisfaction scores when compared to primary arthroplasty outcomes.
