Abstract
In a longitudinal analysis in which 40 to 65-year old male participants of the Prospektive Cardiovascular Münster (PROCAM) study with no history of myocardial infarctions or strokes at the start of the study were kept under observation for four years, there were 55 myocardial infarctions or deaths from coronary heart disease within these four years, while 2132 subjects survived the period without coronary heart disease (CHD). Thrombin-clottable fibrinogen was 24,7 mg/dl higher in the men who developed CHD than in those who did not (p < 0,01). The incidence of CHD in the upper tertile of the fibrinogen distribution was three times higher than in the lowest tertile. In a bivariate and in a multiple logistic regression model taking into account age, systolic blood pressure, cigarette smoking, diabetes mellitus, attacks of angina pectoris and family history of myocardial infarction, the regression coefficient of fibrinogen was statistically significant. In a model that additionally included cholesterol and HDL cholesterol, the coefficient did not approach statistical significance (coefficient 0,00286, standard deviation 0,00189, p = 0,13). The incidence of CHD rose as fibrinogen increases within the middle and particularly the upper tertile of cholesterol distribution.
Zusammenfassung
In einer Längsschnittauswertung von 40- bis 65jährigen männlichen Teilnehmern der Prospektive Cardiovascular Münster (PROCAM) Studie, die zu Beginn der Studie noch keinen Herzinfarkt oder Schlaganfall erlitten hatten und einheitlich vier Jahre nachbeobachtet wurden, traten 55 Herzinfarkte oder koronare Todesfälle innerhalb des Beobachtungszeitraumes auf, während 2132 Probanden den Zeitraum von vier Jahren ohne Inzidenz einer koronaren Herzkrankheit (KHK) überlebten. Die Fibrinogenwerte lagen bei Probanden mit KHK im Mittel um 24,7 mg/dl höher als bei Probanden ohne KHK (p < 0,01). Die Inzidenzrate im obersten Terzil des Fibrinogen war etwa dreimal so hoch wie im untersten Terzil. Sowohl in einer bivariaten als auch in einer multiplen logistischen Regressionsanalyse war der Regressionskoeffizient des Fibrinogen signifikant von Null verschieden, sofern der Einfluß der Variablen Alter, systolischer Blutdruck, Rauchen, Diabetes mellitus, Angina pectoris und familiäre Belastung kontrolliert wurde. Bei zusätzlicher Berücksichtigung von Cholesterin und HDL(High density lipoprotein)-Cholesterin in der multiplen Regression wurde das Signifikanzniveau nicht mehr erreicht (Koeffizient: 0,00286, Standardabweichung: 0,00189, p = 0,13). Die Inzidenz der KHK stieg mit wachsendem Fibrinogen innerhalb des mittleren und vor allem des oberen Cholesterinterzils an.
