The term glomus-tumor is incorrect and should be eliminated in clinical use. We are concerned with tumors of the paraganglia of a. carotis and v. jugularis which are called non-chromaffin paraganglioma. The term “non-chromaffin” (Lattes and Waltner, 1949) may be dispensed with since the chromaffin tumors of the suprarenal gland marrow are designated as phaechromocytoma and never as paraganglioma. The term “chemodectome” (Mulligan, 1950) is not comprehensive enough, as only few paraganglioma deliver humorally active substances—and, as the chemoreceptive activity of the tympanic paraganglia could not be proved yet.

With respect to origin and morphology the paraganglioma are a homogeneous family. In the clinical point of view (bilateral and familiarly frequent occurrence, sexual dimorphism and malignancy) the paraganglioma caroticum, tympanicum and vagale, however, are very different and show a definite independence.

The criteria of malignancy of a paraganglioma jugulare-tympanicum are, in analogy to recent terminology of the cylindrome = adenoidcystic carcinoma, briefly discussed. After a period of 15 to 20 years both tumors will lead to a patient's death. Yet, do paraganglioma of the middle ear partically never metastasise in contrast to the adenoidcystic carcinoma (cylindroma). Therefore, it is widely suggested that in such a case one should not speak of a carcinoma.

Die Bezeichnung Glomustumor ist falsch und sollte auch im klinischen Gebrauch gestrichen werden. Es handelt sich um Tumoren der Paraganglien von A. carotis und V. jugularis, also um nicht chromaffine Paragangliome. Auf das Epitheton „nicht chromaffin” (Lattes u. Waltner, 1949) kann verzichtet werden, da die chromaffinen Tumoren des Nebennierenmarkes als Phaeochromocytom bezeichnet werden und nie als Paragangliom. Der Begriff „Chemodektome” (Mulligan, 1950) ist nicht umfassend genug, da die wenigsten Paragangliome humoral aktive Substanzen abgeben — und die chemoreceptorische Tätigkeit der tympanalen Paraganglien nicht erwiesen ist.

Hinsichtlich Herkunft und Morphologie sind die Paragangliome eine einheitliche Familie. In klinischer Hinsicht (bilaterales und familiar gehäuftes Auftreten, Sexualdimorphismus und Bösartigkeit) sind das Paraganglioma caroticum, tympanicum und vagale jedoch sehr verschieden und weisen eine definierte Eigenständigkeit auf.

Die Kriterien der Bösartigkeit eines Paragangliom jugulare-tympanicum werden, in Anlehnung an die neuere Terminologie des Zylindroms = adenoidcystisches Carcinom, kurz diskutiert. Beide Tumoren fühfren nach 15–20 Jahren zum Exitus des Patienten. Doch metastasieren Paragangliome des Mittelohres praktisch nie, im Gegensatz zu den adenoidcystischen Carcinomen. Es wird deshalb die Meinung vertreten, daß man hier nicht von Carcinomen sprechen sollte.
