Summary
Simultaneous measurements of the intraocular pressure in 7 nonoperated eyes with open-angle glaucoma and the values of acid-base balance in arterial blood were performed after injection of 500 mg acetazolamide (Diamox). In another experimental series on 11 eyes the metabolic acidosis induced with Diamox was buffered by NaHCO3. The intraocular pressure without buffering was lowered to 38.9% and the rate of aqueous flow to 56%. After buffering the intraocular pressure was lowerd to 37.8% and the rate of aqueous flow was reduced to 55%. These results demonstrate clearly that the metabolic acidosis induced by Diamox plays no important role in depression of the aqueous secretion. From clinical point of view, it should be emphasized that equilibration of the acid-base balance is absolutely essential.
Zusammenfassung
An 7 unoperierten, an einem Glaucoma simplex leidenden Augen von Patienten mit einem ungestörten Säure-Basenhaushalt wurden nach intravenöser Injektion von 500 mg Azetazolamid (Diamox) gleichzeitig Bestimmungen der arteriellen Blutgaswerte und Messungen des intraocularen Druckes (AT) vorgenommen. In einer weiteren Versuchsreihe, die 11 nach den gleichen Kriterien ausgewählte Augen umfaßte, wurde die durch Diamox induzierte metabolische Acidose mittels Na-Bicarbonat-Infusionen abgepuffert. Ohne Pufferung bewirkte Diamox eine maximale Senkung des intraocularen Druckes von 38,9% und eine Hemmung der Kammerwasserproduktion um 56%, nach Pufferung eine maximale Drucksenkung von 37,8% und eine Hemmung der Kammerwasserproduktion um 55 %. Diese Ergebnisse beweisen, daß einer metabolischen Acidose keine ins Gewicht fallende Bedeutung für den drucksenkenden Effekt von Acetazolamid zukommt. Aus der Sicht des Klinikers ist ein Ausgleich der oft beträchtlichen und nicht unbedenklichen Verschiebung des Säure-Basenhaushaltes durch eine entsprechende Zufuhr basischer Valenzen zu fordern.
