Summary
Biopsies of two patients with a scleromyxedema Arndt-Gottron (generalized lichen myxedematosus) have been examined electron-microscopically. The patients showed the typical clinical lesions and a paraproteinemia, but actually no other signs of a multiple myeloma. In the superficial and central parts of the corium, connective tissue cells and mast cells are increased. Many of them show a thickened outer layer of the unit-membrane. The contact between cell protrusions and collagen fibrils is unusually close. Isolated fibrils or groups of two or more collagen fibrils are surrounded by the cell; eventually some fibrils seem to be located intracellularly. This lesion is interpreted as a more intensive destruction of collagen fibrils than normally. The success of immunosuppressive therapy in a case recently described could be another indication of an autoaggressive mechanism in scleromyxedema Arndt-Gottron. Reticular and especially elastic fibres are increased in scleromyxedema. The great number of tubular formations also indicates an activated elastogenesis. The increased amount of ground substance, rich in acid mucopolysaccharides, is not only demonstrated by the modified Hale-technique for electron-microscopy, but also by conventional fixation and contrasting methods.
We are of the opinion that the cutaneous lesion in scleromyxedema is the result of the paraproteinemia. Comparative examinations of other cutaneous mucinoses are necessary to prove the specificity of these findings for scleromyxedemaArndt-Gottron.
Zusammenfassung
Hautbiopsien von zwei Patienten mit einem klinisch voll ausgeprägten, mit einer Paraproteinämie aber ohne weitere Anzeichen für ein multiples Myelom einhergehendem Skleromyxödem Arndt-Gottron wurden in verschiedener Weise für die elektronenmikroskopische Untersuchung verarbeitet. In der sog. Ablagerungszone, d. h. in den oberen und mittleren Anteilen des Coriums findet man eine Vermehrung der Bindegewebs- und Mastzellen. Häufiger als in normaler Haut sieht man Verdickungen des äußeren Blattes der “unit-membrane” dieser Zellen. Ungewöhnlich ist die enge Kontaktnahme zwischen Zellausläufern und kollagenen Fasern. Die kollagenen Fibrillen werden einzeln, paarweise oder zu kleinen Gruppen geordnet von der Zelle umschlossen und liegen teilweise wahrscheinlich sogar intracellulär. Wir glauben, daß es sich dabei um einen vermehrten Faserabbau handeln könnte.
Auf das Vorliegen eines Autoaggressionsmechanismus beim Skleromyxödem Arndt-Gottron weist auch ein kürzlich mitgeteilter Erfolg unter einer immunosuppressiven Behandlung hin. In vermehrtem Maße als normal sind elektronenoptisch retikuläre und vor allem elastische Fasern zu sehen. Auf eine verstärkte Elastogenese weisen auch die zahlreichen in Haufen angeordneten Tubuli hin. Die vermehrte, an sauren Mucopolysacchariden reiche Grundsubstanz konnten wir nicht nur mit der für die Elektronenmikroskopie modifizierten Haleschen Reaktion sondern auch mit konventionellen Fixation-und Kontrastierungsmethoden dekorieren bzw. darstellen. Die Hautveränderungen beim Skleromyxödem Arndt-Gottron hängen sicher mit der Paraproteinämie zusammen, die als primärer Faktor angesehen werden darf. Vergleichende Untersuchungen bei anderen cutanen Mucinosen sind notwendig um abzuklären, ob die hier beschriebenen Befunde für das Skleromyxödem Arndt-Gottron spezifisch sind.
