Summary

A brief survey of the organisation in the United Kingdom of existing facilities for rehabilitation, training and sheltered employment precedes a description of five years work in a voluntary, non-profit making organisation (Bristol Industrial Therapy Organisation (Bristol) Limited). The classification of patients referred during this period is reported and the outcome of attendance. The success and failure, according to sources of referral and diagnosis, is commented on and a two-year follow-up is given. A summary of outcome for the first eleven years of the Organisation is recorded. — The present state of industrial therapy is discussed and the need for research and for workers in the field to become more highly specialised is stressed. — The change in clientele is noted with an increase in referral of patients suffering from subnormality. A likely increase in persons with personality disorders is anticipated. — The different methods of approach by I. T. O. to the problem of reemployment are enumerated and the applicability of the I. T. O. principle to other groups, for example, ex-prisoners, prisoners, physically handicapped, etc. is suggested. The strength accruing to the Organisation from close links with the hospital and community is noted and the Organisations view that there should not be separate rehabilitation services for the psychiatrically ill and for the physically ill is stated. The continuation of voluntary organisations such as I. T. O. is considered desirable but not in the continuous provision of services which are clearly the responsibility of statutory authorities. These authorities must give fresh thought to the problem of providing services appropriate to modern needs.

Une brève revue de l'organisation, au Royaume Uni, des possibilités existantes de réhabilitation, de formation et d'emploi protégé précède la description d'un travail de 5 ans dans une organisation volontaire sans but lucratif (Bristol Industrial Therapy Organisation (Bristol) Limited). On expose la classification des patients qui lui ont été adressés pendant cette période, ainsi que le résultat de la fréquentation de cette organisation. Les succès et les échecs sont commentés selon les diagnostics et on donne une catamnèse de 2 ans. Un résumé des résultats des 11 premières années de l'organisation est présenté. On discute l'état actuel de la thérapie industrielle et on insiste sur le fait qu'il est nécessaire que la recherche et les travailleurs dans ce domaine deviennent plus hautement spécialisés. On remarque un changement dans la clientèle de l'organisation dans ce sens qu'on lui adresse davantage de patients qui souffrent de “sub-normalité”. On prévoit une augmentation semblable de personnes souffrant de troubles de la personnalité. Les différentes méthodes par lesquelles la I. T. O. approche le problème du réemploi sont énumérées et on suggère la possibilité d'appliquer les principes de la I. T. O. à d'autres groupes, par exemple aux anciens détenus, aux détenus, aux handicapés physiques, etc. On constate la force acquise par l'organisation grâce aux liens étroits qu'elle entretient avec l'hôpital et la communauté et on confirme l'opinion de l'organisation selon laquelle il ne devrait pas y avoir de services de réhabilitation séparés pour les malades psychiatriques et pour les malades physiques. Il est considéré comme souhaitable que des organisations volontaires comme la I. T. O. poursuivent leur activité, mais pas en continuant à fournir des services qui sont clairement du ressort des autorités. Ces dernières doivent reconsidérer le problème de la prévision de services appropriés aux besoins modernes.

Zusammenfassung

Ein kurzer Überblick über die Organisation von bestehenden Einrichtungen für Rehabilitation., Ausbildung und beschützte Arbeit in Großbritannien geht der Beschreibung einer 5-jährigen Arbeit in einer freiwilligen gemeinnützigen Organisation (Bristol Industrial Therapy Organisation (Bristol) Limited) voraus. Es wird über die Klassifikation der innerhalb dieses Zeitraums überwiesenen Patienten und über den Erfolg ihres Besuchs berichtet. — Erfolg und Mißerfolg werden im Hinblick auf die Überweisungsquelle und die Diagnose diskutiert. Außerdem wird über eine Nachuntersuchung nach zwei Jahren berichtet. Für die ersten 11 Jahre des Bestehens der Organisation wird das Ergebnis zusammenfassend berichtet. — Der gegenwärtige Stand der Arbeitstherapie wird diskutiert und der Bedarf an Forschung und an besser spezialisiertem Personal auf diesem Gebiet wird hervorgehoben. — Eine Veränderung in der Klientel wird zusammen mit einer Zunahme der Überweisung von geistig behinderten Patienten beobachtet. Eine Zunahme von Personen mit Persönlichkeitsstörungen wird prophezeit. — Die unterschiedlichen Umgangsmethoden der I. T. O. mit dem Problem der beruflichen Wiedereingliederung werden aufgezählt und die Anwendbarkeit des I. T. O. -Prinzps auf andere Gruppen, z.B. ehemalige Strafgefangene, Gefängnisinsassen, Körperbehinderte usf. wird empfohlen. Es wird vermerkt, daß die Organisation aus ihrer engen Bindung an Krankenhaus und Gemeinde starken Zuwachs erfährt. Außerdem hat die Organisation die Meinung, daß es keine getrennten Rehabilitationsdienste für psychisch Kranke und für körperlich Kranke geben sollte. Der Fortbestand von freiwilligen Organisationen wie der I. T. O. wird als wünschenswert betrachtet, aber nicht in Form einer stetigen Bereitstellung von Diensten, die eindeutig in den Verantwortungsbereich der Behörden gehören. Diese Behörden müssen sich neue Gedanken über das Problem einer an moderne Bedürfnisse angepaßten Entwicklung von Diensten machen.
