La glicemia e l'insulina immunoreattiva (IRI) sono state misurate, nel corso delle 24 h, prima ed esattamente 1 h dopo glucosio, in soggetti sani cui venivano somministrati, dopo una notte di digiuno, tre carichi di glucosio (50 g p.o.) ad uguali intervalli di tempo, ovvero alle ore 8, 16 e 24. Il test era ripetuto tre giorni più tardi, ma il primo carico di glucosio veniva somministrato alle ore 16, dopo 14 h di digiuno, e così via agli stessi intervalli di tempo. È stato rilevato che la variazione diurna della tolleranza glicidica può dipendere strettamente dalla durata del digiuno e che non esiste un ritmo circadiano. Al contrario, la risposta insulinemica allo stimolo glicemico è sempre più elevata al mattino. Tale comportamento potrebbe essere in accordo con l'ipotesi di una ritmica variazione della secrezione dell'insulina, indipendente dalla durata del digiuno ma collegata a fattori ancora sconosciuti.
Résumé
La glycémie et l'insuline immunoréactive (IRI) ont été mesurées, au cours des 24 hrs, avant et 1 h exacte après glucose, chez des sujets sains auxquels on administrait, après une nuit de jeûne, trois surcharges de glucose (50 g p.o.) à intervals reguliers, c'est-à-dire à 8, 16 et 24 h. L'épreuve était répétée trois jours plus tard, en administrant la première surcharge de glucose à 16 h, après 14 hrs de jeûne, et ainsi de suite avec les mêmes intervals. Il a été observé que la variation de la tolérance au glucose au cours de la journée peut dependre d'une façon étroite de la durée du jeûne et qu'il n'existe pas un rhythme circadien. Au contraire, la réponse de l'insuline au stimulus glycémique est toujours plus élevée le matin. Ce comportement pourrait être en accord avec l'hypothèse d'une variation rhythmique de la sécrétion insulinique, indépendante de la durée du jeûne mais en rapport avec des facteurs encore inconnus.
Resumen
Se han medido la glicemia y la insulina inmunoreactiva (IRI) durante las 24 hrs antes y exactamente 1 h después de glucosa, en individuos sanos a los que se les había suministrado, tras una noche de ayuno, tres cargas de glucosa (50 g p.o.) a intervalos de tiempo idénticos, es decir a las 8, a las 16 y a las 24 hrs. Tres días más tarde fue repetido el test pero la primera carga de glucosa se suministró a las 16 hrs, tras 14 hrs de ayuno, y así sucesivamente respetando los mismos intervalos de tiempo. Se ha observado que la variación diurna de la tolerancia glicídica puede depender estrictamente de la duración del ayuno y que no existe un ritmo circadíano. Por el contrario, la reacción insulinémica al estímulo glicémico es siempre más elevada por la mañana. Dicho comportamiento podría concidir con la hipótesis de una variación rítmica de la secreción de insulina, independiente de la duración del ayuno, pero relacionada con factores todavía desconocidos.
Zusammenfassung
Blutzucker und immunreaktives Insulin im Plasma (IRI) wurden während 24 Stunden bei gesunden Versuchspersonen vor und genau eine Stunde nach der Verabreichung von Glukose bestimmt. Die Versuchspersonen erhielten nach der nächtlichen Fastenperiode drei gleiche Mengen Glukose (50 gper os) in gleichen Zeitabständen, nämlich um 8 h, 16 h und 24 h. Die Probe wurde drei Tage später wiederholt, wobei jedoch die erste Glukosebelastung um 16 h nach 14 Stunden Fasten verabreicht wurde, gefolgt von den weiteren Mengen in den gleichen Zeitabständen. Es wird gezeigt, dass die täglichen Schwankungen der «Glukosetoleranz» weitgehend von der Dauer der vorhergehenden Fastenperiode abhängig sind und keinen wirklichen zirkadianen Rhythmus aufweisen. Dagegen ist die Insulinreaktion auf den Blutzuckerreiz am Morgen immer höher als zu anderen Tageszeiten. Dieses Verhalten kann einer rhythmischen Variation der Insulinsekretion im Laufe einer 24-Stundenperiode zugeschrieben werden und ist daher nicht von der Dauer des Fastens abhängig sondern von bis jetzt noch unbekannten Faktoren.
Summary
Blood sugar (B.S.) and plasma immunoreactive insulin (IRI) were measured before and exactly 1 h after the administration of glucose during a 24-h period in healthy subjects receiving, after an overnight fast, three constant glucose loads (50 gper os) at constant time intervals, i.e. at 8 a.m., 4 p.m. and 12 p.m. The test was repeated three days later but the first glucose load was given at 4 p.m. after a fourteen-hour fast and so on at the same intervals. Some evidence is given that the diurnal variation of «glucose tolerance» is strongly dependent on the duration of fasting and does not exhibit a true circadian rhythm. Conversely, the insulin response to glycemic stimulus is always higher in the morning. This pattern is compatible with a rhythmic variation during a 24-h period of insulin secretion, not depending therefore on the duration of fasting but on as yet unknown factors.
