Unusual mechanism of pathogenicity of pseudomonas aeruginosa isolates from patients with cystic fibrosis

Ein ungewöhnlicher Pathomechanismus eines Pseudomonas aeruginosa Stammes von einem Patienten mit zystischer Fibrose

Summary

APseudomonas aeruginosa strain was isolated from a patient with cystic fibrosis and was found capable of growing within polymorphonuclear leucocytes. As illustrated by electron microscopy, several mechanisms accounted for the intracellular growth: (a) Cell division occurred within the lysosomes; (b) The fusion of the phagosome with the primary lysosome was inhibited; (c) The phagosomal membrane disintegrated, and the bacteria grew within the cytosol; (d) Cytosolic material was separated from the surroundings by bacterial membranes, and subsequently the bacteria coated with host material were released from the leucocyte. The patient suffered from several severe acute pulmonary exacerbations as long as the intracellularly growing variant was detectable in his sputum. After the strain had been eliminated by antibiotic therapy the patient improved (from stage two to one).

Zusammenfassung

Von einem Patienten mit zystischer Fibrose (CF) wurde einPseudomonas aeruginosa-Stamm isoliert, der sich in den polymorphkernigen neutrophilen Granulozyten zu vermehren vermag. Elektronenmikroskopische Aufnahmen belegten, daß mehrere Mechanismen für das intrazelluläre Wachstum verantwortlich sind: (a) Die Bakterien teilten sich in den Lysosomen. (b) Die Fusion des Phagosoms mit dem primären Lysosom fand nicht statt. (c) Nach Zerstörung der Phagosomenmembran vermehrten sich die Bakterien im Zytosol. (d) Das Bakterium umgab zytosolisches Material der Wirtszelle mit einer Membran, und anschließend wurde der Keim aus den Leukozyten freigesetzt. Der CF-Patient litt an schweren rezividierenden Pneumonien, solange der in den Leukozyten überlebende Keim im Sputum nachweisbar war. Nachdem der Keim mit einer hochdosierten antimikrobiellen Therapie eliminiert worden war, verbesserte sich der klinische Zustand des Patienten vom Stadium II zum Stadium I.
