Zusammenfassung
Bei 56 Angehörigen diabetischer Kinder wurden die Nüchternkonzentrationen von Glucose, freien Fettsäuren und Insulin im Serum untersucht. Gegenüber stoffwechselgesunden Kontrollpersonen fand sich bei den kindlichen Probanden eine Erhöhung der Serumglucose, jedoch keine Abweichung der FFS- und IMI-Konzentrationen. Bei den erwachsenen Probanden wurden dagegen im Mittel Erhöhungen aller 3 Parameter gegenüber den Kontrollgruppen nachgewiesen. Den statistischen Normbereich (M±2σ der Vergleichsgruppe) überschreitende gleichzeitige IMI- und FFS-Konzentrationserhöhungen fanden sich nur bei 3 von insgesamt 56 Probanden. Qualitative Analysen der einzelnen Fettsäurenfraktionen im Serum ließen bei diabetischen Kindern, ihren Angehörigen und genetisch nicht belasteten Kontrollpersonen eine im wesentlichen übereinstimmende prozentuale Verteilung erkennen.
Fehlende Konstanz der gefundenen Normabweichungen bei Kontrolluntersuchungen schränkt den Wert der einmaligen Untersuchung der erwähnten Parameter für die Diagnostik latent diabetischer Stoffwechselsituationen erheblich ein.
Summary
Fasting concentrations of serum glucose, free fatty acids, and immunoreactive insulin were measured in 56 relatives of diabetic children. In sisters and brothers an elevation of glucose, but no differencies in FFA and insulin were demonstrated, compared to non-diabetic controls. Adult subjects showed mean elevations of all three parameters. Simultaneous elevations of insulin- and FFA-concentrations, surpassing the normal range, calculated as mean ±2 SD of the control group, however, only occurred in 3 of 56 subjects.
A qualitative analysis of different serum FFA-fractions by gas-chromatography demonstrated a comparable composition of circulating FFA in diabetic children, their non-diabetic relatives and normal children, respectively.
Since the reported deviations could not regularly be demonstrated in repeated investigations of the same individual, measurements of fasting concentrations of glucose, FFA, and insulin apparently are of limited value in screening programs for latent diabetes.
