Abstract

(1)
The strains occurring in the subchondral bone of the cadaver human femoral head were measured when the head was subjected to a load of physiological magnitude and direction.

The strains were tensile around the circumference of the femoral head and compressive in the plane of the applied load.

In the intact femoral head the magnitude of the strains was such that fatigue fracture of the subchondral bone would not be expected to occur in life.

When a cylindrical defect 13 mm in length and 10 mm in diameter was made in the centre of the femoral head, the measured strains in the subchondral bone invariably rose, and in some femoral heads the calculated stresses were high enough to be likely to cause fatigue fracture.

It is concluded that cysts developing in the femoral head in osteoarthrosis may usually result in a rise in the stresses in the subchondral bone, with a possible consequent revascularization of the overlying cartilage either via a fatigue crack or by reason of the vascular changes accompanying the healing of an incipient fatigue crack. The probability of either of these events cannot be assessed accurately with the information available, but it appears to be low.

Lorsqu'un défaut cylindrique de 13 mm de long et 10 mm de diamètre a été pratiqué au centre de la tête femorale, les contraintes mesurées dans l'os subchondral augmentaient invariablement et dans certaines têtes fémorales les efforts calculés étaient suffisamment grands pour causer une fracture par fatigue.

Il est conclu que des kystes se formant dans la tête fémorale en ostéo-arthrose peuvent normalement mener à un accroissement des tensions dans l'os subchondral avec une revascularisation conséquente possible du cartilage superficiel soit par une craquelure de fatigue soit du fait des changements vasculaires qui accompagnent la guérison d'une craquelure de fatigue incipiente. La probabilité de l'un ou de l'autre de ces évènements ne peut être estimée avec précision avec les renseignements disponibles, mais elle semble être faible.

Die Spannungen, welche in dem subchondralen Knochen im Leichnam eines menschlichen Oberschenkelkopfes vorkommen, wurden gemessen, als der Kopf einer Belastung physiologischen Ausmasses und Richtungsweise unterworfen wurde.

Es ergaben sich Zugspannungen rund um die Oberfläche des Oberschenkelkopfes und Druckspannungen in der Ebene der angewandten Belastung.

In dem unbeschädigten Oberschenkelkopf war die Grösse der Spannungen derart, dass Ermüdungsbruch in dem subchondralen Knochen nicht zu Lebzeiten zu erwarten gewesen wäre.

Als eine zylindrische 13 mm lange Beschädigung von einem Durchmesser von 10 mm im Mittelpunkt des Schenkelkopfs durchgeführt wurde, stiegen die gemessenen Spannungen in dem subchondralen Knochen beständig an, und in einigen Oberschenkelköpfen waren die berechneten Spannungen gross genug, um wahrscheinlich Ermüdungsbruch hervorzurufen.

Man kommt zu dem Schluss, dass Zysten, die sich bei Osteoarthrose entwickeln, gewöhnlich eine Steigerung der Spannungen in dem subchondralen Knochen hervorrufen können, mit einer möglicherweise folgenden Gefässneubildung des darüberliegenden Knorpels, entweder durch Ermüdungsbruch oder aus Gründen von Gefässänderungen, welche die Heilung eines beginnenden Ermüdungsbruchs begleiten. Die Wahrscheinlichkeit dieser beiden Vorkommnisse kann auf Grund der vorhandenen Kenntnisse nicht genau bestimmt werden, sie erscheint aber gering.
