Zusammenfassung
Wir führten eine prospektive randomisierte Untersuchung zum Vergleich der kombinierten Zytokintherapie mittels Interferon alfa-2b, Interleukin 2, und 5 Fluorouracil vs. einer Misteltherapie bei 74 Patienten mit metastasiertem Nierenzellkarzinom durch. Im Therapiearm A erhielten die Patienten eine kombinierte Immuntherapie mittels Interferon alfa-2b (IFN alfa-2b s.c.; 4,5×106 IE/m2 an Tag 1 der Wochen 1 und 4 und an Tagen 1, 3 und 5 der Wochen 2, 3; 9×106IE/m2 an den Tagen 1, 3 und 5 der Wochen 5 bis 8), Interleukin-2 (IL-2 s.c., 18×106 IE/m2 an Tagen 3, 4 und 5 der Wochen 1 und 4 sowie 4,5×106 IE/m2 an Tagen 1, 3 und 5 der Wochen 2 und 3) und 5-Fluorouracil (5-FU i.v. 1×750 mg/m2 pro Woche der Wochen 5–8). Im Therapiearm B erhielten die Patienten 1 ml Mistellektin s.c. 2×pro Woche. Im Therapiearm A entwickelten 4 Patienten eine komplette Remission (CR) und 7 eine partielle Remission (PR). Darüber hinaus fand sich bei 11 Patienten eine stabile Erkrankung (SD 30%) und bei 15 Patienten schritt die Erkrankung unter Therapie fort (PD 40%). Im Therapiearm B zeigte sich keine Remission (0/37), 6 Patienten zeigten 2 Monate nach Therapiebeginn Bild gebend eine stabile Erkrankung (SD 16%) und bei 31 Patienten wurde nach 2 Monaten Misteltherapie ein Progress nachgewiesen (PD 84%). Bedingt durch die Möglichkeit des Cross over sind die Überlebenszeiten basierend auf der initialen Randomisierung nicht aussagekräftig. Es fand sich ein medianes Überleben von 5 Monaten für Patienten des Therapiearms B ohne Cross over (3–11 Monate). Das mediane Überleben für Patienten des Therapiearms A lag mit 18 Monaten (1–102+ Monate) deutlich höher. Trotz größerer Nebenwirkungen durch die kombinierte Zytokintherapie war die Rate des Therapieabbruchs in beiden Gruppen gleich.
Abstract
We performed a prospective randomized study to compare combined cytokine therapy consisting of interferon-α2b, interleukin-2, and 5-fluorouracil with mistletoe therapy in 74 patients with metastatic renal cell cancer. The patients in treatment arm A received combined immunotherapy with interferon-α2b (IFN-α2b s.c. 4,5×106 IU/m2 on day 1 of weeks 1 and 4 and on days 1, 3 and 5 of weeks 2, 3, and 9×106IU/m2an days 1, 3 and 5 of weeks 5–8), interleukin-2 (IL-2 s.c. 18×106 IU/m2 on days 3, 4, and 5 of weeks 1 and 4 and 4.5×106 IU/m2 on days 1, 3 and 5 of weeks 2 and 3), and 5-fluorouracil (5-FU i.v. 1×750 mg/m2 per week in weeks 5–8). The patients in treatment arm B received 1 ml mistletoe lectin s.c. two times per week. Four patients in treatment arm A achieved complete remission (CR) and seven patients partial remission (PR). In addition, the disease was stable in 11 patients (SD 30%) and progressed under therapy in 15 (PD 40%). There were no cases of remission in treatment arm B (0/37); imaging study 2 months after start of therapy revealed stable disease in six patients (SD 16%) and progressive disease in 31 patients (PD 84%) after 2 months of mistletoe therapy. Due to the possibility of crossover, survival times based on the initial randomization are not significant. The median survival was 5 months in patients in treatment arm B without crossover (3–11 months). The median survival of patients in treatment arm A was clearly higher at 18 months (1–102+ months). Despite considerable side effects caused by the combined cytokine therapy, the dropout rate was the same in both groups.
