Achtergrond

Sinds 1998 is het mogelijk om de huisartsopleiding te combineren met promotieonderzoek als ‘arts in opleiding tot huisarts en onderzoeker’ (aiotho). Ons doel was in kaart te brengen hoe het aantal aiotho’s zich heeft ontwikkeld en hoeveel zij bijdragen aan de wetenschappelijke ontwikkeling van de huisartsgeneeskunde.

Methode

In de database van het landelijk aiothonetwerk bepaalden wij het totaal aantal aiothotrajecten tussen 1 januari 1998 en 1 juli 2012. In PubMed inventariseerden wij voor die periode het aantal huisartsgeneeskundig relevante publicaties waarin aiotho’s een aandeel hadden, en op basis van NHG-gegevens gingen wij na hoeveel aiotho’s deelnamen aan NHG-richtlijnwerkgroepen. De huidige werkzaamheden van oud-aiotho’s brachten wij in kaart met behulp van een vragenlijst.

Resultaten

Op 1 juli 2012 bevatte de database 81 aiotho’s en 34 oud-aiotho’s, naast 16 ex-aiotho’s die het onderzoek of de opleiding voortijdig beëindigd hadden. Van de oud-aiotho’s waren er 33 (96%) werkzaam als huisarts en 13 (39%) als onderzoeker. Van de ex-aiotho’s die voortijdig met hun onderzoek gestopt waren, was het merendeel werkzaam als huisarts, geen enkele als onderzoeker. Zowel het aantal gestarte als het aantal met succes afgeronde aiothotrajecten toonde een globaal stijgende lijn, tot respectievelijk 11 en 7 in 2011. Dat gold ook voor het aantal huisartsgeneeskundig relevante onderzoeksartikelen in PubMed met een aiotho als (co)auteur: 80 in de eerste helft van 2012. Aiotho’s hadden zitting in de richtlijnwerkgroepen van 13 (15%) NHG-richtlijnen die op 1 juli 2012 gepubliceerd waren, en van 6 (38% ) richtlijnen die op dat moment in herziening waren.

Conclusie

Aiotho’s leveren een groeiende bijdrage aan de wetenschappelijke ontwikkeling van de huisartsgeneeskunde in Nederland. Om uitval uit het promotietraject te verminderen zou meer aandacht kunnen uitgaan naar een goede onderzoeksopzet, een reële planning en goede begeleiding.

Background

Since 1998, medical students have been able to combine their vocational training in general practice with research leading to a PhD degree, the so-called ‘aiotho’ training programme. The aim of this study was to investigate the further careers of a number of these aiotho students and how they have contributed to the development of general practice.

Method

Using the database of the national aiotho network, data from 1 January 1998 to 1 July 2012 were retrieved on the number of students who had started and completed the aiotho programme, the number of articles in PubMed relevant to general practice published by former aiotho students, and the number of former aiotho students who had contributed to the development of guidelines published by the Dutch College of General Practitioners (NHG). A questionnaire was used to determine the current activities of former aiotho students.

Results

On 1 July 2012, the database contained information on 81 aiothos and 34 former aiothos; 16 other students had stopped their study prematurely. Of the former aiotho students, 33 (96%) worked in general practice and 13 (39%) in research. None of the students who stopped their study prematurely were in research; most were general practitioners. The number of students who had started and completed their study increased from 1998 to 2012, by 11 and 7 per year, respectively. The same was true for the number of relevant publications with former aiotho students as author or co-author: 10 in 2000 and 80 in the first 6 months of 2012. Former aiotho students were involved in the publication of 15% of the NHG guidelines and in 38% of the guidelines currently under revision.

Conclusion

Since the aiotho programme started in 1998, the number of students who started and completed the programme, the number of relevant publications by former aiotho students, and the number of former aiotho students who participated in the development of NHG guidelines have increased. The aiotho programme appears to deliver a substantial contribution to the development of general practice medicine and primary care in the Netherlands.
