Résumé
L’évaluation de l’état nutritionnel de la personne âgée en réanimation est une tâche difficile, souffrant de l’absence de référentiel. Les marqueurs biologiques classiques et anthropométriques sont de peu d’utilité. La prévalence de la dénutrition et de la sarcopénie est importante en réanimation. La sarcopénie est prédictive de la mortalité à court et long terme et doit être dépistée et traitée. L’utilisation du Subjective Global Assessment (SGA) et du Nutrition Risk In Critically Ill Score (NUTRIC score), ainsi que la recherche de la présence d’une sarcopénie par CT-scan coupe en L3 paraissent répondre aux objectifs d’évaluation nutritionnelle de la personne âgée en réanimation.
Abstract
Assessment of nutritional status of aged adults in ICU is very difficult because of lack of recommendations. Usual human and biomarkers are unuseful. Denutrition and sarcopenia are frequent in ICU. Sarcopenia predicts mortality and must be diagnosed and treated. Subjective global assessment (SGA), Nutrition Risk In Critically Ill Score (NUTRIC score), and CT Scan in the 3rd lumbar vertebra (L3) could be the objective tools for assessment of the nutritional status of aged adults in the ICU.
