Zusammenfassung
Die Sprache, derer sich Klienten und/oder Therapeuten in einer Sitzung bedienen, hat das Potenzial, klinisch bedeutsame Prozesse offenzulegen. Daher hat sich die Textanalyse von Sitzungstranskripten als wertvoller Ansatz erwiesen, um ein besseres Verständnis für die Funktionsweise von Psychotherapie zu entwickeln. Ein computergestütztes Modell, das sich zu diesem Zweck eignet, ist das therapeutische Zyklusmodell. Ziele dieser Arbeit sind die Betrachtung des theoretischen Hintergrunds und empirischer Daten des Zyklusmodells sowie die Erarbeitung von Anregungen für zukünftige Forschungen. Emotionale und abstrakte Wörter der Klienten und Therapeuten werden automatisch in den Transkripten der Sitzungen evaluiert und zur Identifizierung verschiedener Modalitäten der emotional-kognitiven Regulierung („relaxing“, „experiencing“, „reflecting“ und „connecting“) des Klienten, des Therapeuten und der Dyade herangezogen. Des Weiteren werden mithilfe einer bestimmten Sequenz dieser emotional-kognitiven Regulierungsmodalitäten schließlich jene Momente, in denen ein Therapiefortschritt stattgefunden hat (therapeutische Zyklen), identifiziert. Aus mehreren empirischen Studien geht hervor, dass zentrale Variablen des Zyklusmodells (Connecting, therapeutische Zyklen) auf klinisch relevante Prozesse während einer Sitzung schließen lassen und sich auf das Therapie-Outcome auswirken könnten. Zukünftige Prozess- und Prozess-Ergebnis-Studien sollten die für das therapeutische Zyklusmodell sprechenden empirischen Belege vertiefen und verdeutlichen.
Abstract
Within-session language used by clients and/or therapists can reveal clinically meaningful processes. For this reason, the text analysis of session transcripts has been shown to be a valuable approach for a better understanding of how psychotherapy works. The therapeutic cycles model (TCM) is a computer-assisted model to be used with this aim. This article reviews the theoretical background and empirical findings of TCM and provides suggestions for future research. Emotional and abstract words used by the client and by the therapist are automatically assessed within session transcripts and used to identify different modalities of cognitive emotional regulation (relaxing, experiencing, reflecting and connecting) of the client, of the therapist and of the dyad. Moreover, a specific sequence of such modalities of emotional cognitive regulation is finally used to identify moments of therapeutic progress (therapeutic cycles). Several empirical studies showed that the key TCM variables (connecting, therapeutic cycles) are indicative of clinically relevant within-session processes and may have an influence on treatment outcome. Future process and process outcome studies should be conducted in order to further expand and elucidate the empirical evidence in favor of the TCM.
