Sphincteroplasty for incontinence after surgery for idiopathic fistula in ano

Abstract.

Between 1990 and 1992, external sphincter repair was performed in 20 patients rendered faecally incontinent after earlier fistula surgery. All were improved, 13 (65%) achieving Grade 1 or 2 continence. Postoperative ultrasound was useful in explaining the sub-optimal outcome of 3 patients who still had defects. Clinical outcome in female patients was better than in male patients. Clinical outcome was not related to the results of preoperative manometry, the site of external sphincter damage, the use of a stoma, or the occurrence of wound complications. External sphincter repair for faecal incontinence after fistula surgery achieves good results in the majority of patients.

Résumé.

Entre 1990 et 1992, vingt patients devenus incontinents suite à la cure chirurgicale d'une fistule anale ont fait l'objet d'une sphinctéroplastie du sphincter externe. Une amélioration a été notée dans tous les cas: treize patients (65%) ont atteint un degré de continence de 1 ou 2. Une échographie endo-anale post-opératoire a été utile pour expliquer les résultats sub-optimaux obtenus chez 3 patients qui avaient toujours des troubles de la continence. Le résultat clinique est meilleur chez les femmes que chez les hommes. Le résultat clinique n'est pas à mettre en relation avec le résultat de la manométrie pré-opératoire ni avec le siège de la lésion sphinctérienne externe ni avec la présence d'une stomie ou la survenue de complications au niveau de la plaie opératoire. La reconstruction du sphincter externe, en cas d'incontinence fécale secondaire à la chirurgie des fistules, entraîne de bons résultats dans la majorité des patients.
