Assoziation zwischen Häufigkeit der Sonographie und Prävalenz bösartiger Neubildungen der Schilddrüse bei Versicherten der AOK in Bayern

Association between frequency of sonography and prevalence of malignant neoplasms of the thyroid in insurees of the AOK in Bavaria

Zusammenfassung

Hintergrund

Bayern weist von allen deutschen Bundesländern die höchste Inzidenz des Schilddrüsenkarzinoms (SDK) auf. Das SDK ist derzeit der häufigste endokrine Tumor und für ca. 2 % aller invasiven Malignome in Bayern verantwortlich. Aufgrund des in den letzten 15 Jahren beobachteten SDK-Inzidenzanstiegs bei gleichzeitig abfallender SDK-Mortalität besteht der Verdacht auf Überdiagnostik mittels Schilddrüsen(SD)-Sonographie.

Methoden

Anhand von Routinedaten der kassenärztlichen Versorgung wurde eine ökologische Studie auf Kreisebene in Bayern zur Untersuchung einer möglichen Assoziation zwischen der Häufigkeit der SD-Sonographie einerseits sowie der SDK-Prävalenz bei allen Versicherten der AOK bzw. der bayernweiten SDK-Inzidenz andererseits im Jahr 2013 stratifiziert nach Geschlecht mittels Korrelations- und Regressionsanalyse durchgeführt. Ergänzend wurde die Häufigkeit sonstiger SD-Diagnostik sowie von SD-Resektionen analysiert.

Ergebnisse

Es bestand eine mäßige Assoziation bei Frauen (Korrelationskoeffizient [rS], 0,40) und Männern (rS, 0,31) zwischen der Häufigkeit der SD-Sonographie einerseits sowie der SDK-Prävalenz andererseits bei AOK-Versicherten in Bayern. Pro 1000 zusätzlich durchgeführte SD-Sonographien stieg die Anzahl prävalenter SDK-Erkrankungen bei Frauen um 5,4 bzw. bei Männern um 4,5 an (95 %-Konfidenzintervall [KI]; Frauen 2,5–8,3; Männer 1,2–7,8). Dies entspricht einem Anstieg der absoluten SDK-Prävalenz um eins pro 185 (Frauen) bzw. um eins pro 222 (Männer) zusätzlich durchgeführte SD-Sonographien. Der Betrag aller anderen untersuchten Assoziationen lag unter 0,2.

Schlussfolgerung

Diese Ergebnisse geben erste Hinweise auf das Vorliegen einer Überdiagnostik als eine der Ursachen des beobachteten SDK-Inzidenzanstiegs bei rückläufiger SDK-Mortalität. Eine Bestätigung dieser Ergebnisse anhand größerer Bevölkerungsgruppen und separat nach Tumorgröße, histologischem Subtyp sowie anhand des zeitlichen Verlaufs der beobachteten Assoziation wird empfohlen.

Abstract

Background

Of all German federal states, Bavaria shows the highest incidence of thyroid cancer. Thyroid cancer is currently the most frequent endocrine tumor and responsible for approximately 2% of all invasive malignant tumors in Bavaria. Due to the increased incidence of thyroid cancer observed over the last 15 years and a simultaneous reduction of mortality due to thyroid cancer, there is the suspicion of overdiagnosis using sonographic examination of the thyroid gland.

Methods

Based on routine data of the statutory healthcare in Germany, an ecological study was carried out at the district level in Bavaria to investigate a possible association between the frequency of thyroid sonography and the prevalence of thyroid cancer in all insurees of the AOK (“Allgemeine Ortskrankenkasse”). Also investigated was the incidence of thyroid cancer throughout Bavaria in 2013 stratified according to sex using correlation and regression analyses. The frequencies of other diagnostic methods for thyroid cancer and thyroid gland resections were also analyzed.

Results

The results showed a moderate association for women (correlation coefficient, rS, 0.40) and for men (rS, 0.31) between the frequency of thyroid sonography and the prevalence of thyroid cancer in AOK insurees in Bavaria. The number of prevalent thyroid cancer diseases in women increased by 5.4 and for men by 4.5 per 1000 additionally carried out thyroid sonographic investigations (95% confidence interval, CI: women 2.5–8.3; men 1.2–7.8). This corresponds to an increase in the absolute prevalence of thyroid cancer of 1 per 185 (women) and 1 per 222 (men) additionally carried out thyroid sonography examinations. The amount for all other associations investigated was less than 0.2.

Conclusion

These results provide preliminary evidence for overdiagnosis as one of the causes of the observed increase in the incidence of thyroid cancer with declining mortality due to thyroid cancer. Confirmation of the results based on larger population groups and differentiation according to tumor size and histological subtype as well as the temporal course of the observed association is recommended.
