(746) - Perceived Health Status in Adult Fontan Patients: The Impact of Previous Heart Failure Hospitalization

Purpose

Patients who undergo the Fontan surgical procedure for complex congenital heart disease now commonly survive to adulthood though remain at risk for complications including heart failure (HF). This study investigated perceived health status among adult Fontan patients and the impact of prior HF hospitalizations.

Methods

In this baseline cohort analysis, adult Fontan patients attending a tertiary adult congenital heart disease clinic were enrolled between 10/2010 and 01/2014 and underwent full clinical evaluation. Perceived health status was assessed with the Short Form-12 Health Status Survey (SF-12). History of HF hospitalization was defined as at least one non-elective hospitalization for HF prior to study entry.

Results

A total of 142 patients with Fontan physiology (54% male; mean age = 27±9 years) completed the SF-12 survey. A history of HF hospitalization was documented in 35 (25%) of patients. Patients with prior HF hospitalization were older (35±11 vs 25±7 years, p<0.0001) and more likely to have a history of arrhythmia (77% vs 50%, p=0.006).Compared to the general US population as well as Fontan patients without a prior history, those with a history of HF hospitalization had significantly poorer health

Conclusion

Prior HF admission is associated with poorer subjective and objective health status among adult Fontan patients. Close surveillance is indicated after Fontan-related HF hospitalization and should incorporate assessment of both physical and psychosocial well-being.
