      Problems of sharing information within and across various organizations involved with health care delivery are addressed in this paper, and the authors propose three possible approaches to solving the problems. Their objective is to implement information system support for the changing healthcare delivery model, so that patient care becomes both efficient and cost-effective while maintaining high quality. It is implied, but not stated explicitly, that implementation of information systems will minimize the variations in treatment costs, and, by implication, the variations in treatment that are currently observed. The bulk of the article details a variety of approaches to the problems as described. This is a technical exposition and proposed solution, based in part on the premise that information systems will be able to address the problems and play a major role in generating solutions. While not inappropriate as a proposal, the reasons for the failure of information systems, to date, to effect the goals described should be made more explicit, especially for those readers who are not fully aware of the long and difficult history of computing in medicine generally, and in relation to medical records in particular: at least 40 years without any obvious real solutions. The proposal for integration is certainly appropriate, but appears to lack universal applicability, given the diversity of health care delivery models between and within countries. It seems unlikely that a single solution will suffice, given the innate variability of the situation. The basic issues must be made explicit, and only then should solutions be proposed. It seems that we lack the details to proceed with universal solutions.
