초록

A quasi-experimental study was conducted to examine the effects of case management for patients with hypertension. Forty subjects were selected from the participants in the patient-tailored home visiting project of the public health centers. Twenty were allocated to the experimental group receiving 8 weeks of case management program which consisted of 6 home visits and 2 telephone calls. The rest 20 received 8 weeks of usual nursing care which consisted of 3 home visits. Significant positive changes were found in the experimental group in knowledge on hypertension (p=.006), self-care for hypertension (p<.001), and systolic blood pressure (p<.001). However, there was no difference in change of cholesterol and triglyceride between two groups. In conclusion, case management in the patient-tailored home visiting project showed positive impact on hypertension management. Various ways to provide continuity of care for the chronic hypertension patient should be explored.
